Request access to a child's or adult's ARC MyChart account
Log in to your MyChart account and follow the steps below.
- Click Your Menu, then click on Family Access & Personalize Your Record under the Sharing section.
- Click the Request access to a minor's record link if you are a parent or guardian. The Request to Access a Minor's Record request form will display.
- Fill out the form and click Submit Request. The request will be reviewed before access is granted.
When access is granted, the parent or guardian will receive a notification in their MyChart inbox.
What you will see when you have child proxy access:
Child proxy access applies to dependent accounts for children between the ages of 0-17, and the information you can access depends on the age of the child.
These age range limitations do not affect any legal right you have to access your child's record by other means. To request a paper copy of your child's record, contact your child's primary care clinic or submit a Release of Information form. Fees and other information can be found here.
- AGES 0-11: You will be granted full access to your child's MyChart record.
- AGES 12-17: Minors have the right to consent to certain health care without parental consent under the Texas Family Code. HIPAA requires the physician to maintain the confidentiality of the care provided to a minor under those conditions. Proxy access for patients in this age group will provide parental access to include the following areas:
- Immunization records
- Appointment scheduling and viewing of future and past appointments
- Lab Results, excluding sensitive test results
- Clinical Messaging
- Billing Summaries for account guarantors
- AGE 18: Your child is an adult, and you will no longer have access to their MyChart record unless they grant you access as an adult proxy (see instructions below).
Requesting direct access to MyChart by a minor
- AGES 15-17: A minor in this age group may have their own individual access to MyChart. Minors have the right to consent to certain health care without parental consent under the Texas Family Code. HIPAA requires the physician to maintain the confidentiality of the care provided to a minor under those conditions. To request this access, please talk to your care team during your next clinic visit.
