ARC Diabetes Program

The ARC Diabetes Program is dedicated to providing you with all the tools you need to gain control of your diabetes. We currently offer two types of diabetes education programs to ensure you get the support that best fits your needs:

  • Diabetes Education Program

    The Diabetes Education Program provides a structured and supportive group setting for our patients.

    During Diabetes Education Program classes, you will learn more about:

    • Being active
    • Healthy eating
    • Managing medications
    • Monitoring blood sugar levels
    • Reducing risks of diabetes
  • Diabetes Intensification Program (DIP)

    DIP offers frequent check-ins and ongoing education from one of our Family Medicine advanced practice clinicians (APCs) who will work closely with your primary care physician or APC to coordinate your care.

    In the DIP program, you will meet with one of our diabetes specialists who will review your medications, make changes where possible, and discuss ways to modify your diet and lifestyle that may help improve your glucose control. During check-ins, you will learn more about:

    • Continuous glucose monitoring (CGM)
    • General diabetes management of type 1 and type 2 diabetes
    • Medication management
    • Ways to better manage A1c

The ARC Diabetes Program is available to ARC patients 18 and older with a referral from your ARC care team.  After an initial assessment with one of our diabetes management educators, you will be accepted into the program that best fits your needs.

FAQs

  • Are there any individual Diabetes Education Program classes available?

    We encourage patients to attend Diabetes Education Program group classes. However, depending on your situation, individual in-person or virtual classes may be an available option. Individual sessions must be requested and approved by your diabetes management educator during your initial assessment.

    If approved for individual in-person or virtual classes, you will be required to attend six classes (1.5 hours each) instead of the standard three in-person classes (2.5 hours each) to complete the Diabetes Education Program.

  • Do I have to attend all Diabetes Education Program classes?

    You must complete an initial assessment and attend all three group classes or all six individual classes in order to complete the Diabetes Education Program.

    Please notify our team if you decide not to complete the program. Otherwise, our team will follow up to see how we can best accommodate your needs.

  • How many people usually attend Diabetes Education Program classes?

    Diabetes Education Program classes typically have 5 to 7 patients per session, depending on available space.

  • How long do Diabetes Education Program classes and DIP check-ins typically last?

    Diabetes Education Program
    Group classes are about 2.5 hours long, with scheduled breaks in between.

    Individual in-person and virtual classes are typically 1.5 hours long.

    Diabetes Intensification Program (DIP)
    DIP check-ins typically last 15 to 45 minutes, depending on your needs.

  • Is the Diabetes Program covered by my insurance?

    Diabetes Education Program

    Most insurances cover the Diabetes Education Program and may require a copay.

    You may need to verify coverage by contacting your insurance company and providing the following billing codes:

    • G0108
    • G0109

    Diabetes Intensification Program (DIP)

    Visits with a DIP specialist are covered with the same copay as a primary care office visit. There is no specialty copay.
