On Thursday, April 10, the America's Physician Groups (APG) Texas Regional Meeting was held at Austin Regional Clinic (ARC) Administration, where medical professionals, administrators, CEOs, and senior leaders from all over Texas gathered to share insight on access, quality, and cost-effectiveness in healthcare.
At the meeting, Manish Naik, MD, Chief Medical Officer and CMIO of ARC, led the welcome and kicked off the meeting with introductions. Later, Gemma A. Borja, Vice President of Population Health & Clinical Quality, and Zeena Abdul-Kafor, Clinical Quality Manager of ARC, highlighted ARC's best practices and STAR performance metrics for improved patient health and engagement.
Meeting attendees learned how ARC achieves above average patient outcomes by leveraging digital tools for scheduling and patient outreach, connecting medically complex patients with personal nurse navigators, and collaborating with local hospitals to receive information about discharged patients.
Below are a few highlights from ARC's presentation:
- ARC's Medicare Share Savings Program ranked in the top 15% of ACOs nationally.
- HPV vaccination rates increased nearly 40% over the last 3 years.
- Childhood immunization rates reached 70%, exceeding the Texas average of 62%.
- Patients with diabetes enrolled in Chronic Care Nurse Navigation saw a 1.2 average decrease in HbA1c levels.
- The outreach team contacted 85K+ patients to schedule preventive care, cancer screenings, and Medicare Wellness visits.
